Instructions for using the spreadsheet

Step 1

Enter your name, BSB, Account Number and the name of your bank.

Step 2

Enter the date the fee was charged as shown on your statement. Then enter the amount of fee in ONE of the fee type columns. These columns are for Bank Accounts only.

Step 3


For Credit Card claims enter the date the fee was charged as shown on the statement. Then enter the amount of fee in the Credit Card fee type column. These colums are for Credit Card Accounts only.

Step 4

The totals will appear in the Sub Total column automatically.

Complete one spreadsheet per account. If you have a bank account and a credit card complete one spreadsheet for each.

Save the spreadsheet. You can now send the completed spreadsheet using EITHER the file upload option when you register, OR after you register using the File Upload After Registration form.

To ensure your uploaded file is allocated to the correct entry on the database we need your unique ID number.

Step 5

A unique ID number was included in the Welcome Email you received when you first registered.  If you can't find it send a request and we will send your number.
